Pest control checklist template

A pest control checklist records the regular checks of traps and bait stations: where they are, any sign of rodents or insects, and what was done. It is not a pest control policy in itself: your policy, or the pest control part of your health and safety and food safety procedures, says what you do, and this template is the record that shows you did it. Cafés, snack bars, food trucks and small restaurants keep it, whether they check the traps themselves or use a pest control company. Many businesses check every week and add the contractor's visits to the same sheet.

What the law requires

Regulation (EC) No 852/2004, Annex II, Chapter I requires food premises to allow good hygiene practice, including protection against pests, and Chapter IX requires adequate procedures to control pests. Article 5 asks for HACCP records that fit the size of the business. In the UK, the Food Standards Agency pack Safer food, better business has the same kind of pest checks. How often you check and what you do when you find pests come from your own HACCP plan and the national guide.

Pest control needs documented procedures. Mark the number and position of each trap on a floor plan.

How to fill it in, in 5 steps

  1. 1Write the business name and the month, and attach a floor plan that shows the number and position of every trap and bait station.
  2. 2Check every trap and bait station on the day set in your plan, and look for droppings, gnaw marks, insects and damaged packaging around the premises.
  3. 3For each trap, write the date, the trap number, what you found and whether the bait was eaten or replaced.
  4. 4If you find signs of pests, protect or throw away affected food, call your pest control company and record the action in the corrective action log.
  5. 5Sign each check, keep the contractor's visit reports with the sheet, and file everything in the HACCP folder.

Questions

How often should pest control checks be done in a restaurant?

Many small food businesses check their own traps every week and have a pest control company visit at set intervals. Your HACCP plan and the contract with the company set the exact frequency.

Do I need a pest control contract for a small café?

Regulation (EC) No 852/2004 requires adequate procedures to control pests, not a specific contractor, but national rules may ask for more. Many small businesses use a pest control company because its visit reports also serve as records.

What should I do if I find signs of mice in my kitchen?

Protect or throw away any food that may be affected, clean the area, call a pest control company and record what you found and what you did.

How long do you keep the logs?

As long as your HACCP plan and the rules in your country require. Regulation (EC) No 852/2004, Article 5, requires documents and records that fit the nature and size of the business.

The templates help you keep records. They do not replace your own HACCP plan.
